How to make a feedback QR code

    1. Create a short formIn Google Forms, add one rating question and one open question. Add others only if you will act on the answers.
    2. Copy the linkClick Send, choose the link icon, tick "Shorten URL" and copy it.
    3. Paste it abovePick a frame that says what you want: "HOW DID WE DO?" works well.
    4. Test as a customerSubmit an answer from a phone that isn't signed in, then check that it reached your sheet.

    Feedback form or Google review?

They do different jobs, and many businesses need both.

    Private feedback formGoogle review
Who sees itOnly youEveryone searching for you
Good forFixing problems, staff coachingNew customers, local search
Needs a Google accountNot if your form allows itYes
Works forAny business, school or eventBusinesses with a Google profile

Be even-handed with reviews. Google's rules prohibit selectively asking only happy customers to post. Offer the review link to everyone rather than filtering people through a form first.

    Questions that get useful answers

Short forms get finished. Start from one of these:

    SettingRating questionOpen question
Restaurant or cafeHow was your meal today? (1 to 5)What should we do better?
Shop or salonHow likely are you to recommend us?What nearly stopped you buying?
ClinicHow was your visit today? Was anything unclear?
Training or workshopHow useful was this session? What would you change?
School or coaching classHow clear was this topic?Which part needs another class?
EventWould you come again? What was the best part?

Make the rating required and the open question optional. Use multiple-choice wherever you can, because typing on a phone is slow.

    Where to place the code

People respond while they wait, so put the code where they have a free moment:

    • Restaurants: on the bill folder or table, not the menu.
    • Clinics and salons: at the waiting area or payment desk.
    • Shops: on the receipt, carry bag or packaging.
    • Training and classes: on the last slide, and leave it up during questions.
    • Events: at the exit, plus an email afterwards.

The printable templates make table tents and stickers, and the bulk QR code generator can make one code per branch or table.

    Compare branches, tables or classes

To see which location each reply came from, add a question such as "Which branch?". Better still, use a pre-filled link so people don't have to answer it. In Google Forms, choose ⋮ → Get pre-filled link, select the branch and copy the link. Make one code per branch from those links.

Keep it anonymous if you can

People are more honest when they aren't identified. In Settings → Responses, set email collection to off. Don't turn on "Limit to 1 response" unless you must, since it forces people to sign in. If you do ask for a phone number to follow up, make it optional and say why.

    Act on it

Read replies weekly and fix one thing. If you change something, say so near the code: "You asked for faster billing, so we added a second counter." It shows that answering is worth it, and more people will.

    Frequently asked questions

How do I make a QR code for a feedback form?

Create a form in Google Forms, copy its link from the Send window, paste it on this page and download the code.

Is a feedback QR code free?

Yes. The code is free with no scan limit, and Google Forms is free with a Google account.

    Can feedback be anonymous?

Yes. In the form settings, turn off email collection and don't ask for names. Avoid "Limit to 1 response", because it makes people sign in.

    Where do the responses go?

To the Responses tab in your form, and to a Google Sheet if you link one. PloraKit never sees them.

Can I use one QR code for many branches?

Yes, with a "Which branch?" question. For more accurate data, make one code per branch using pre-filled form links.

Can I change the questions after printing?

Yes. Editing the form keeps the same link, so the printed code keeps working.
